STEP 1 - SUBMIT THE APPLICATION FORM ONLINE

 

STEP 2 - WAITING FOR APPROVAL

Please wait for the confirmation. Our team will review your application form, and send the result to your email. 

Once we approve your application, you will receive (1) online signing emails for the Release and Waiver Form and Vendor Agreement and (2) online payment email. 

 

STEP 3 - ACCEPT THE OFFER

To accept the offer, you will need to complete (1) signing documents online, and pay the invoice for the vendor fee.

• Vendor Agreement
• Release and Waiver  

Payment checklist: (Choose one payment method from below)

• Pay it Online (card processing fee 4%)
• Mail a Check
